Please join us on Wednesday 15th October as we present a session on Transition from Paediatric to Adult Rehabilitation Services by Dr Jayasri Srinivasan & Evelyn Culnane via MS Teams.
Dr Srinivasan is a Consultant Physician (Paediatric Neurology & Rehabilitation) at Victorian Paediatric Rehabilitation Service, The Royal Children’s Hospital Melbourne. She is also an Honorary Fellow at the Murdoch Children’s Research Institute & Honorary Clinical Lecturer for the Department of Paediatrics at University of Melbourne.
Evelyn leads the Transition Support Service at the Royal Children’s Hospital (RCH) Melbourne, which in 2020 received the inaugural Exemplar Practice Award from the Australian Commission on Safety and Quality Health Care. The Service is an integral hospital-wide service which cares for over 2000 patients annually across all paediatric and adult health service disciplines, general practice and the community.
